Doctor David Callaway, a recent widow, desperately tries to renew contact with his 9-year-old daughter, Emily, after his wife's tragic death. After several months of psychiatric treatment with Dr. Katherine Carson, one of David's former students, Emily seems to be on the road to overcoming the tragedy and seems ready to get on with living. David, her father, decides to leave New York, preferring a small town life and devoting time to his father-daughter relationship. But things take a turn for the worse when Emily creates a fantastic and scary imaginary friend, "Charlie", who loves to play gruesome games.
